New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.
If you live outside of our practice area, we would be able to register you but as an 'out of area patient'.
This entitles you to access to all our services, except for home visits and you will need to sign a form stating you agree to those terms. This will not be a suitable arrangement for you if you would not be able to come in to the surgery.
Registration forms are available to collect in our reception area, upstairs in the Health Centre, first window.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. We also require one form of photographic ID when you hand the forms in.
All new patients between the ages of 40 and 74 will be offered a health check with a member of the healthcare team at some stage following registration to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
