Dear Patient,
We would like to know how we can improve our service to you and how you perceive our surgery and staff.
To help us with this, we have set up a patient representation group so that you can have your say. We ask the members of this group some questions from time to time, such as what you think about the range of services we offer and the times at which you are able to access them, or your thoughts about the quality of the care or service you received. We also undertake a patient survey from time to time so we can measure in a more scientific manner what you think about the service we offer. Contact is normally by email with links to our surveys so it shouldn’t take too much of your time.
We have been running a survey for some months for members of the group and we have published the results in the Survey Results section. We are now running a consultation with members of the group on the results of the survey. We will publish the results of that in the near future.
We aim to gather a group of patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups. So if you are interested in helping us understand what is important to you please join the group. There is a link below.
If you are happy for us to contact you occasionally by email please click the link below to open the sign-up form and complete all the fields.
If you prefer, you can download the sign up form as a pdf document, print it out, complete it and return it to the practice.
We will be in touch shortly after we receive your form. Please note that no medical information or questions will be responded to.
Many thanks for your assistance
